Student Athlete Medical Clearance Requirements

Immunizations
It is required by Rutgers University-Camden to have updated immunization records on file. All students and student-athletes must upload their immunization records to the Rutgers University-Camden Wellness Center portal.

Sickle Cell Trait
Sickle cell trait testing/results is required by the NCAA for all student athletes. Sickle cell trait results can be obtained by the hospital at which you were born OR online from the state of which you were born. If you were born in New Jersey, you can call the NJ State Department of Health at 609-530-8371. If you were born in Pennsylvania, you can call the PA State Department of Health at 717-783-8143. This process can take an extended period of time to obtain information. Please do this step ASAP. If you are unable to obtain your sickle cell results, contact your primary care physician for more information regarding sickle cell testing/results, or contact the Rutgers University-Camden Student Wellness Center to schedule an appointment for Sickle Cell testing. Once sickle cell results are obtained, student-athletes are responsible for uploading their results to their ATS profile.                 *Student-athletes will not be allowed to participate in their sport until Sickle Cell testing results are acquired and uploaded to the student-athletes' ATS profile.

Physicals
All student-athlete physicals must be performed by the student-athlete's primary care physician. Student-athletes will be required to print the Rutgers University-Camden NCAA Pre-Participation Physical Exam form, which is linked here:

Upon completion of the pre-participation physical exam form, student-athletes are required to upload their completed physical to their ATS profile. (Please refer to Rutgers University-Camden Student Athlete Pre-Participation information packet for ATS profile instructions)
                *Any student athlete who does not have a primary care physician may take their printed pre-participation physical form to the Rutgers University-Camden Student Wellness Center to fulfill their medical physical requirements.

RU-C Athletic Training Mandatory Administrative Meeting Day
All student-athletes will be required to attend their mandatory team meeting day scheduled with the athletic training staff. This is when you will complete baseline concussion testing and compliance meetings. Any student-athlete that is unable to attend their respective team's scheduled day, is to contact the athletic training staff ASAP and head coach.

**Any student-athlete that has a medical condition/injury/surgery/illness/ongoing medical issue (i.e. heart murmur, sickle cell trait, concussion) that requires another doctor's clearance for participation must note this information in their medical history and bring all required documents with them to their sports physical.
