Rutgers-Camden Athletics

Prior to your tryout, we ask that you complete the following checklist of items.  Please read and sign all forms in ink and they must be legible!   Without that paperwork your physical is considered incomplete and you will be unable to participate.  If you are a minor (under the age of 18), please make sure that you go over the forms with your parents and all forms are signed where applicable. 

The following items are REQUIRED by all student-athletes looking to participate in a tryout.
 

• Fill out the following form online with your demographic information and medical history information.  If a field does not apply, please put (-). The form can be found at:
  • INCOMING ATHLETE FORM

 

• Schedule a physical exam at Health Services. Physicals done by your Primary Care Physician will NOT be accepted. Health Services can be contacted at 856-225-6005. You will need:
  • Complete Immunization Record
  • Documentation of your sickle cell trait status. This can be obtained from your primary care physician (at time of birth).  If you are unable to obtain your records, Health Services will test you free of charge.
    • If you were born in the state of New Jersey please complete the following form: NJ Sickle Cell Screening Request Form
    • If you were born in the state of Pennsylvania, please complete the following form: PA Sickle Cell Screening Request Form
    • If you were born in the state of New York, please complete the following form: NY Sickle Cell Screening Request Form
  • Consent to Treat Signed by parent if under the age of 18: Consent to Treat Form

 

•  Print, read and sign the following Forms:
  • Athletic Training Rules and Regulations

Additional paperwork is required if you make the team. The necessary paperwork will be communicated to you.
